Requesting Patient Records

We leverage a third-party service called ChartPro to fulfill requests for patient records. If you call our office to request a copy of a patient’s records, you’ll be connected to ChartPro, or you can call ChartPro directly at 770‐434‐0042. Patients can also complete the medical records release form and submit it to ChartPro to request a copy of their records.

ChartPro keeps track of all the records requests received, when the request was received, and when the documents were produced. Georgia Urology works with ChartPro to fulfill those requests in a timely manner.
